How to improve Quality according to DPMO?

I have an exam tomorrow and although I feel quite prepared, I am still struggling with one bit.
I know how to implement the outcomes from SERVQUAL or House of Quality because I can talk about specific measures that the company should take in order to improve the results.
But for example, if I calculate the DPMO (defects per million opportunities), and I get a result which is really far a away from 3,4 (Six Sigma), what should I recommend to the company? There are so many TQM tools and techniques so I don't really know how to focus on the most important ways for them to reduce their DPMO and reach Six Sigma?!
